Case History:
- A 65-year-old male patient underwent surgical drainage of a amoebic cyst in the left lobe of the liver.
- After 2 uneventful days, the patient is readmitted with massive haemorrhage through the drainage catheter.
The patient is referred for emergent angiography.
3 different phases during selective left hepatic artery angiography.
- Active extravasation (contrast media outside the normal arterial network with progressive deformation and increasing size )
- Feeling of the abscess with active bleeding
Which treatment?
- Embolization with:
- Glue
- Coils
- Gelfoam
Our approach:
- Super-selective catheterisation with a microcatheter
- Embolization with 3 torpedos of gelfoam (Gelitaspon)
- Final result shows no more active bleeding
- Patient recovers and drainage is withdrawn successfully 3 weeks later
