15th February 2010 - Endovascular Forum presents a GEST Interactive Case: Embolization and Massive Traumatic Retroperitoneal Hemorrhage
A 44-year-old man was airlifted to the hospital after a severe motor vehicle accident. CT scan demonstrated pelvic fractures and free extravasation of iodinated contrast into the left retroperitoneum. He was very hypotensive despite rapid transfusions of blood products, intravenous fluids, and a cell saver. Interventional radiology was consulted for urgent control of his hemorrhage.

Congratulations to Ziv J. Haskal, MD, FSIR, FAHA, FACR, an Endovascular Forum Steering Committee member, on publication of his study this week in the New England Journal of Medicine. Dr. Haskal is Professor of Radiology and Surgery, and Chief of the Vascular and Interventional Radiology Division at the University of Maryland Medical Center in Baltimore, MD. He is also co-founder of the Global Embolization Symposium and Technologies (GEST) scientific meeting. |

Dr. Haskal and colleagues report the results of a multicenter trial in which patients with a prosthetic hemodialysis graft experiencing venous anastomotic stenosis underwent balloon angioplasty alone compared to balloon angioplasty plus placement of a stent graft. This is the first large, randomized, controlled trial to demonstrate success over balloon angioplasty alone. The investigators reported a highly statistically significant greater incidence of patency in the stent-graft group and freedom from subsequent interventions at 6 months. Binary restenosis was approximately one-third that of angioplasty. Although balloon angioplasty is the current standard for venous anastomotic stenosis, patients still develop restenosis within a week or months following the procedure. An estimated 120,000 balloon angioplasties are performed every year in the United States for dialysis graft patients.
